Agreements and Enrolment Documents

Congratulations on receiving your conditional offer for the PG Diploma in Forensic Psychology Practitioner course. Before we can confirm your unconditional place, there are a few essential documents that must be completed and returned to us.

What you need to do

1. Download the documents below, which include all five required agreements/forms.

To be completed by you, the applicant:

• The Trainee Health Declaration - Please return this form prior to your interview taking place
• The Trainee Supplementary Information Form
• The Placement Trainee Agreement

To be completed by the clinical supervisor:

• The Placement Provider Agreement
• The Placement Audit Document
• The Placement Risk Assessment Document
• The Placement Health and Safety Induction Checklist - Please return this form prior to your interview taking place.

2. Complete all documents in full. Please ensure every section of each form is filled out accurately and completely. Incomplete forms will delay confirmation of your place. Please note that some forms need to be completed by you, the applicant, and some need to be completed by your clinical supervisor. Please make sure the right form is completed by the right person.

Your offer remains conditional until we receive and process your completed documents. To secure your place, please return the forms as soon as possible. Delays in submission may impact your eligibility to register for the course.
