If you are under 19 years old and in full time education you are automatically exempt from charges. Once you reach 19, you will without exceptions have to pay these costs.
To see if you qualify for a reduction in charges or complete exemption, please obtain a HC1 form from Student Health, complete the form and return it in the pre-paid envelope supplied. Obtain a FP57 (NHS receipt) from the chemist when you pay for your prescription, then obtain a HC5 form (claiming money back) from the local benefits office. If you get an exemption certificate, you can claim your money back.
Where is the chemist?
The nearest chemist is Lloyds Pharmacy, Broadgate Lane (off Town Street) in Horsforth. [map]
